Thanks for providing that screenshot. Since it contained personal info I removed it from the public post and made it only available to forum admins.

The information looked correct, I have escalated the issue to our My Viasat app team so that they can fix the issue on their side. I will let you know when I hear back from them.

Were you using the Safari browser to complete this registration form? Could you please try from a different browser?
